Job Summary
Design and implement Population Health priorities targeting specific patients to improve care quality and cost. Support execution of programs covering admission prevention, transitions-in-care, pharmacy adherence, and medical cost management. Collaborate with Treehouse members, clinicians, and regional leaders to drive strategy and ensure effective implementation of clinical and business priorities. Lead communication on key metrics including quality, utilization, and financial measures while building relationships with functional leaders in Product, Finance, and Data Science. Manage multiple priorities to solve difficult problems and develop business cases for Oak Street's value-based care model.
Required Qualifications
- 5+ years of applicable experience in consulting / healthcare / population health / working with healthcare providers; or other similar fields
- Bachelor's degree required
