Job Summary
Manage Integrated Home Care Services' key health plan relationships by serving as the liaison between clients and internal stakeholders to drive operational excellence and ensure successful service delivery across Home Health, PDN, LTSS, and DME. Facilitate quarterly business reviews and strategic alignment plans while collaborating cross-functionally with Clinical Operations, IT, and Finance to support client initiatives and onboarding. Analyze claims, authorization trends, and utilization patterns to identify improvement opportunities and provide data-driven recommendations to leadership. Oversee complex client implementations, track KPIs and SLAs, and resolve performance issues in accordance with contract terms and regulatory guidelines. Manage special initiatives and pilots to ensure successful implementation and outcome measurement while fostering long-term partnerships.
Required Qualifications
- A bachelor's degree in relevant field such as business, healthcare, marketing, or communications
- 5+ plus years of experience in health plan client success
- Strong operational knowledge of healthcare payers and providers
- Strong relationship-building and strategic partnering skills
- Strong analytical ability to solve business and technical problems
- Solid history of establishing and meeting work schedules with limited time frames under tight deadlines
- Excellent communication and interpersonal skills
- Experience with project management and the ability to manage complex projects
- Attention to detail and the ability to manage multiple clients and tasks simultaneously
- Experience using CRM or customer success software
