Job Summary
Conduct direct support management for up to 60 individuals with developmental disabilities, overseeing residential, day, and respite care services. Formulate and update Person-Centered Individualized Support Plans and risk plans, while attending quarterly meetings and responding immediately to incidents such as falls, elopement, or abuse. Manage Direct Support Professional staff through recruitment, interviewing, hiring, training, and annual reviews. Document incident reports within 24 hours and coordinate with regulatory bodies including DDS, CARF, and CMS. Schedule work times, maintain compliance with Indiana Administrative Code, and utilize AccelTrax and BDS portals for data entry and quality assurance.
Required Qualifications
- Knowledge of Indiana Administrative Code (IAC) 460-6

- Ability to collect documentation from other care providers relevant to the delivery of direct supports
- Ability to upload and download documentation to the Bureau of Disability Services (BDS) online portal
- Ability to respond, investigate, document, and resolve any and all complaints received from individuals supported, or anyone making the complaint on behalf of the individual supported
- Ability to educate individuals and their family on how to report a complaint
- Ability to attend all individual specific quarterly meetings
- Ability to formulate, distribute, and data compilation of an Annual Survey of Individual Satisfaction
- Ability to communicate with individuals supported or individuals communicating on behalf of individuals supported
