Job Summary
Manager of Community Outreach Programs to lead and coordinate outreach programs including EPICC, CBHL, CCP, Housing Liaison, Disease Management, and Integrated Health. The role supports and mentors program staff, ensures safety and regulatory compliance, strengthens partnerships with healthcare providers and community organizations, and monitors program performance with quality improvement initiatives. Responsibilities include tracking outcomes, maintaining accurate documentation, and providing reports to leadership and funders. Requires a Bachelor’s degree in Social Work, Psychology, Sociology, Public Health, Healthcare Administration, or related field; 3–5 years of experience in community or healthcare programs; knowledge of Joint Commission standards, risk management, and regulatory compliance; and a valid Class E driver’s license.
Required Qualifications
- Bachelor’s degree in Social Work, Psychology, Sociology, Public Health, Healthcare Administration, or related field
- 3 to 5 years of experience within community or healthcare programs, care coordination, or behavioral health services
- Knowledge of Joint Commission standards, risk management, and regulatory compliance
- Valid Class E driver’s license and ability to transport participants if required
