Job Summary
Manage programs and clinicians providing primary care to SCAN members within the integrated care delivery model. Oversee clinical team performance to achieve defined outcomes, ensuring adherence to ethics and quality standards for Advanced Practice Providers. Select, onboard, train, and develop clinicians while communicating job expectations, planning, and monitoring results through coaching and counseling. Provide clinical care including exams, diagnostic ordering, care plan development, and patient education. Collaborate with senior management on operational issues and new program development, while improving APP functions and systems across specialties. Develop success metrics to improve program efficiencies and network with internal and external stakeholders to promote membership growth and quality outcomes. This role requires a Master's degree in nursing, Oregon RN/NP licensure, specialty certification, and 5+ years of NP experience with 3+ years supervisory experience. Demonstrated experience with Medicaid and Medicare is essential. Base salary ranges from $125,400 to $215,975 annually.
Required Qualifications
- Master's degree from an accredited school of nursing
- Oregon RN, NP, and Furnishing licensure
- Certification in area of specialty
- 5+ years of recent NP experience
- Demonstrated experience in working with both Medicaid and Medicare
- Strong strategic thinking skills to facilitate the development and delivery of new clinical products and services to seniors and the community
- Strong interpersonal skills, excellent written and oral communication skills
