Job Summary

Lead and coach a team of Kidney Care Specialists supporting individuals with Chronic Kidney Disease and End Stage Renal Disease across field and virtual settings. Oversee navigation services, education on CKD progression, and dialysis access placement while ensuring consistent execution of workflows with physician practices and community locations. Own regional performance outcomes for early CKD education adoption, referral volume, and patient readiness metrics by analyzing trends and guiding strategic resource deployment. Partner with clinical leaders, operations, and market development to align growth strategies, manage complex stakeholder dynamics, and optimize performance across large geographic territories. Track referral trends, engagement rates, and market activity to inform coaching and strategy adjustments. Requires occasional evening and weekend work, travel to physician groups and healthcare locations, and a bachelor's degree or three to five years of clinical background.

Required Qualifications

  • High School diploma or G.E.D.
  • 2+ years of people management/leadership experience
  • Minimum of 3–5 years of experience in kidney care, chronic disease management, healthcare outreach, or related field
  • Valid driver's license with ability to travel within assigned market
  • Must be willing to work occasionally in the evening and weekends, as needed

Desired Qualifications

  • Bachelor's degree preferred
  • 3-5 years clinical background preferred
  • Relevant licensure or certification preferred, as applicable
  • Demonstrated success leading teams accountable for growth, adoption, or performance outcomes
  • CKD education, dialysis, or transplant program experience
  • Prior experience leading community-based or market-facing teams
  • Background in program expansion, transformation initiatives, or growth strategies
  • Familiarity with value-based care models and the role of education in improving outcomes
