Job Summary
Coordinate initiatives to integrate health and social support systems, enhancing community care delivery and population health. Develop and operationalize plans to empower communities for self-care, implementing preventive health strategies across precincts. Track progress on community health plan implementation and manage stakeholder partnerships with grassroots organizations, social agencies, and healthcare providers. Provide secretariat support for meetings and lead projects assigned by the Reporting Officer or Head of Department.
Required Qualifications
- Bachelor degree in any discipline
- Minimum 5 years of working experience
- Able to think critically
- Strong planning, project management and organisational skills
- Adept in stakeholder engagement to foster buy-in and collaboration
- Exceptional written and verbal communication skills
- Resourcefulness in problem solving and resilient in overcoming challenges
