Job Summary
The Manager of Case Management is responsible for overseeing case management processes, collaborating with interdisciplinary teams, and ensuring timely discharge planning for patients. This role requires participation in performance improvement activities and supervision of staff. Candidates must have a Bachelor's degree in Nursing or a Master's in Social Work, with significant experience in case management and direct supervisory roles. The position also necessitates appropriate licensing and certification upon hire.
Required Qualifications
- Bachelor's Degree in Nursing (BSN) or Master's Degree in Social Work (MSW)
- Three (3) years of experience in case management or relevant nursing experience
- Two (2) years of direct supervisor/manager experience
- Licensed in the Commonwealth of Virginia to practice as a Registered Nurse (RN)
- Basic Life Support (BLS) certification
- One of the following certifications: ACM, CCM, CCTSW, or licensed as a Social Worker (LCSW) in Virginia
Desired Qualifications
- Five (5) + years of previous Inpatient (hospital) case management experience
- Two (2) years supervisory/lead experience
- Previous experience working through medically complex cases
- Current holder of ACM or CCM, or LCSW
Additional Requirements
- Must be licensed in the Commonwealth of Virginia to practice as a Registered Nurse (RN) upon start
