Job Summary
Conduct specialized care consultations and needs assessments for ALS caregivers and families facing complex situations. Manage a portfolio of clients to ensure their needs are met while overseeing field staff in assigned territories. Provide information on programs, services, and educational resources, and coordinate referrals with healthcare providers and community agencies. Evaluate support group effectiveness and ensure quarterly engagement with the ALS community. Document all pertinent engagements and staff interactions per Care Services Standards. Serve as a liaison between the ALS Clinic multidisciplinary team and the Association, developing clinic relationships. Educate staff and healthcare professionals on ALS management, research, and association services through in-service programs.
Required Qualifications
- Bachelor's Degree or related experience in social work, case management, health administration, allied and public health, nursing, or a related area
- Minimum of Four years of related professional experience and/or training, or equivalent combination of education and experience
- Demonstrated ability to utilize management skills such as problem-solving, solid reasoning, critical thinking, budget/program planning, performance improvement, organizational planning, implementing, and monitoring performance standards
- Knowledge of Medicare, Medicaid, patient assessments skills, planning and case management

- Experience with progressive neurodegenerative disease
