Job Summary
Manager Care Management position in Sheboygan County seeking an RN or Social Worker leader to oversee daily care management and social work operations at the site. Responsible for staff scheduling oversight, performance improvement activities, data collection/analysis to measure care process effectiveness, regulatory compliance, budget development and control,跨-site collaboration with physician leadership, and ensuring high-quality, cost-effective coordinated care. Requires strong leadership, budget and personnel management, knowledge of Medicare guidelines, Utilization Management, Discharge Planning, and ability to travel across multiple sites in IL/WI if needed. Licensure/certification within 1 year (ACM/CCM/CCCM/RN-BC/C-SWHC as applicable) and a Bachelor’s in Nursing or Master’s in Social Work are required. Start date and schedule: full-time, day shift, 40 hours/week; on-call potential; benefits eligible.
Required Qualifications
- Registered Nurse license issued by the state in which the team member practices, or Social Worker license issued by the state in which the team member practices
- RN: Nursing Case Management (RN-BC) or CCM/ACM certifications within 1 year OR CCM/ACM certifications within 1 year OR ACM/CCM/ACM certifications within 1 year
- Bachelor's Degree in Nursing or Master’s Degree in Social Work
- Typically requires 5 years of experience in a relevant clinical setting with 1 year supervisory experience in a Care Management Leadership role
- Ability to travel and work across multiple sites as assigned (IL or WI)
