Job Summary
Lead a team of nurses, social workers, and care coordinators managing Medicaid member needs in the Illinois market by assessing requirements, guiding families toward resources, and facilitating interactions with community extenders. Oversee multidisciplinary operations to address social determinants of health, monitor program performance through data analysis, and drive continuous improvement while ensuring compliance with federal and state regulations. Foster partnerships with community-based organizations and aging services to enhance care delivery and support initiatives for housing, food insecurity, and transportation. Directly manage up to 15 associates, develop policies for consistent service quality, and lead cross-functional efforts to align departmental processes with enterprise objectives.
Required Qualifications
- Must reside in Illinois or a border state
- Bachelor's degree
- Active Registered Nurse (RN) license or Social Work (e.g., LCPC or LCSW) license
- 5+ years of professional experience
- 2+ years of management or supervisory experience
- Knowledge of IL Medicaid guidelines, benefits and policies and procedures
- Strong understanding of care management models and the role of extenders in addressing social needs
- Proficiency in analyzing and interpreting data trends
- Progressive operational leadership experience
- Demonstrated ability to lead cross-functional initiatives and collaborate with external partners
- Ability to operate independently and in a team environment
- Valid state driver's license
- Personal vehicle liability insurance

- Wireless, wired cable or DSL connection
- Dedicated space lacking ongoing interruptions
- Interagency Language Rating (ILR) test competency for non-English speakers
- TB screening clearance
Desired Qualifications
- Advanced degree in nursing, health field, or business/healthcare administration
- Previous experience working in a managed care field
- 5 or more years of previous management/supervisor level experience
- Experience managing or collaborating with community health workers, peer support specialists, or housing programs
