Job Summary
Oversee two departments within Indiana Lakes Accountable Care Organization, managing chronic care risk modeling and implementing focused case and disease management strategies. Collaborate with primary care physicians, leadership, case managers, and patients to improve care transitions and communication of patient plans across the continuum. Facilitate quality improvement plans with the Patient Center Medical Home Committee and establish relationships with community providers while managing administrative, financial, and regulatory requirements for Business Health Advantage direct contracted services. Requires a Bachelor's in Nursing with an active Indiana license, five to eight years of clinical experience, and proficiency in statistical analysis and database software.
Required Qualifications
- Bachelor's in Nursing
- Active Indiana license as a Registered Nurse
- Five to eight years of clinical experience
- Clinical skill knowledge
- Strong analytical and critical thinking skills
- Understanding of population health principles
- Demonstrate understanding /application of research methodology
- Statistical analysis
- Understanding of medical terminology
- Ability to take initiative and meet objectives
- Knowledge of computer use
- Various data base software programs
- Ability to adapt and be flexible with frequent change
- Strong organizational and interpersonal skills
Desired Qualifications
- Master's in Nursing or related health care field
- Three to five years of experience within healthcare operations or managed care environment
