Job Summary
Coordinate treatment plans and support patients with complex psychiatric and forensic backgrounds in housing, finances, and social network integration. Facilitate discharge preparation, organize external support, and conduct systemic interviews alongside the lead therapist. Maintain regular contact with patients, referral sources, and external providers while contributing to team development and departmental improvements. Work within a multidisciplinary team at the Forensic Psychiatric Unit in Den Dolder, focusing on safe reintegration into society.
Required Qualifications
- afgeronde hbo-opleiding Maatschappelijk Werk en Dienstverlening (MWD), Social Work met uitstroomprofiel Zorg of GGZ Agoog, of Sociaal Pedagogische Hulpverlening (SPH)
- ruime ervaring met patiënten met complexe psychiatrische problematiek
- kennis van actuele maatschappelijke ontwikkelingen
- in staat om jouw expertise duidelijk in te brengen binnen een multidisciplinair team
- affiniteit met de forensische psychiatrie
