Job Summary
Perform focused wound assessments, administer dressing changes and routine wound treatments, and deliver Ultramist therapy as part of Innovative Wound Solutions' field-based LVN wound care team serving Riverside and San Bernardino Counties. Work under physician/APP supervision to execute wound care plans, identify changes in wound status, educate patients and families on wound care goals and signs of complications, collaborate with physicians, NPs/PAs, RNs, and facility staff, and maintain accurate documentation in the EMR, including uploading wound photos and measurements. Represent the organization professionally in patient and facility interactions and travel throughout the specified counties.
Required Qualifications
- Graduate of an accredited LVN program
- Active California LVN license in good standing
- Current CPR certification (AHA BLS)
- Minimum of one year of clinical LVN experience; post-acute or outpatient setting preferred
- Prior wound care experience preferred; Ultramist training will be provided
- Strong clinical documentation skills and attention to detail
- Excellent communication and interpersonal skills with patients, families, and interdisciplinary teams
- Ability to work independently in the field with minimal direct supervision
- Reliable transportation and valid California driver's license required
- Ability to travel throughout Riverside and San Bernardino Counties
