Job Summary
Administers skilled nursing care under the supervision of the RN for patients in their homes, assists with the plan of care, completes clinical notes, and coordinates care with the interdisciplinary team and family. Requires LVN licensure, Houston-area residence travel capability, and a flexible schedule to provide home-based, end-of-life–care support.
Required Qualifications
- Current state license as a Licensed Vocational Nurse
- Graduate of an accredited school of vocational nursing
- Valid and current driver's license
- Reliable transportation
- Ability to travel locally
- Proof of Hepatitis acceptance or declination
