Job Summary
Conduct rounds to observe, assess, document, and report resident conditions, ensuring care plans are implemented as directed. Administer medications per prescription and perform narcotic reconciliation between shifts. Identify significant changes in resident status, initiate incident and accident reports, and manage discharges per facility policy. Oversee nursing aides and complete nursing assessments before making physician calls. Communicate resident status updates to point of contacts. Assist in providing quality care promoting physical, mental, and psychological well-being. Requires active Texas LVN license with skilled nursing facility experience preferred.
Required Qualifications
- Has completed an LVN program at an accredited vocational school or college.
- Must be an active Licensed Vocational Nurse in the state of Texas in good standing.
- Experience working in a skilled nursing facility is preferred.
