Job Summary
Conduct rounds to observe, assess, document, and report resident conditions while administering medications as prescribed. Oversee aides, ensure care plan implementation, and perform narcotic reconciliation between shifts. Identify significant resident changes, initiate incident reports, and manage discharges per facility policy. Must complete nursing assessments before physician calls and communicate status updates to resident contacts. Requires active Texas LVN licensure and skilled nursing facility experience.
Required Qualifications
- Has completed an LVN program at an accredited vocational school or college.
- Must be an active Licensed Vocational Nurse in the state of Texas in good standing.
- Must be able to lift 1-25 lbs

- Must be able to carry 1-25 lbs

Desired Qualifications
