Job Summary
LVN/RN Charge Nurse overseeing nursing care in a post-acute setting with responsibilities including development and implementation of individualized care plans, medication administration, IV therapy within RN scope, patient education, documentation in EMR, shift-change reporting, and collaboration with the health care team. Requires high school diploma or equivalent, graduation from an accredited RN program, valid LVN/RN license in the state, and CPR certification; one year of nursing experience in SNF preferred. Shift is 6am-10pm with weekend coverage and includes a weekend differential.
