Job Summary
The Nursing Coordinator role focuses on medical case management within community behavioral health and crisis programs for participants, operating as part of an integrated mental and health care team. Responsibilities include triaging participants at intake and throughout respite stays, administering psychotropic medication injections, coordinating medications and insurance liaising, providing education and medical information to staff and participants, aiding in care planning with PCPs, documenting all interactions accurately, fostering collaborative teamwork, developing procedures for monitoring antipsychotic medications, and performing other duties as assigned. The position is based in Oakland with a 40-hour full-time schedule and weekend availability, including a Night Shift (NOC) from 12:00am to 8:30am Thursday through Monday.
Required Qualifications
- LVN or LPT certification from accredited nursing school and current CA license
- Background and experience in mental health
- Bilingual English and Spanish desirable
- Valid California driver’s license, personal vehicle, and proof of personal vehicle insurance coverage; DMV printout required
