Job Summary
Conduct healthcare system navigation for patients discharging from hospitals or acute care facilities to ensure smooth transitions. Manage incoming referrals for post-acute care services, including transcription and routing to appropriate lines like Transitional Dialysis and Case Management. Provide telephonic coverage and support for inbound calls, emails, and HIPAA-compliant SMS platforms regarding Transitions of Care, Medical Respite, and appointment scheduling. Assist hospital staff and community partners in securing medical care and facilitate home health referrals, DME needs, and expedited MAP enrollment. Educate discharge planners and physicians on Central Health program criteria while practicing AIDET during interactions. Ensure all patient care and administrative processes comply with regulatory standards, State Board of Nursing requirements, and departmental protocols.
Required Qualifications
- Graduation from an accredited school of Vocational School of Nursing
- 2 years Experience working in a community-based organization, ambulatory clinic, home health, or hospital setting
- Texas Licensed Vocational Nursing License
- Basic Life Support (BLS) obtained through approved American Heart Association or Association of Red Cross
- Must live in the Austin, Texas area
- Must be able to work both at an approved off-site location and onsite at a primary location or multiple locations based on business needs
