Job Summary
Licensed Practical/Vocational Nurse provides direct patient care under the supervision of a registered nurse, implementing the plan of care, documenting accurately, educating patients and families, coordinating home health services with the team (HHA, OT, PT, MSW, ST, Dietitian), and communicating with the office team. Requires graduation from an accredited practical nurse/vocational nursing program, current state LPN/LVN license, CPR certification, a valid driver’s license, and at least three years of nursing experience with prior community/home health or medical/surgical experience; role supports full-time home-health patient care with standard nursing duties and compliance with Code of Conduct.
Required Qualifications
- Graduate of an accredited practical nurse or vocational nursing program
- Currently licensed as a licensed practical nurse or licensed vocational nurse in the state
- Possesses and maintains CPR certification
- Has three years nursing experience and one year prior professional nursing experience in Community/home health or medical/surgical experience
- Must be a licensed driver with a valid automobile
