Job Summary
Provide nursing services in home/community settings for individuals with brain injury, IDD, SUD, or behavioral disabilities. Administer medications and document accordingly, oversee staff who administer medications, conduct health assessments, implement healthcare services in each Individual Service Plan (ISP), respond to condition changes, train staff on universal precautions, infection control, and medication administration, and liaise with medical providers and external agencies to coordinate care. Also provide training on self-medication, sex education/STD prevention, and smoking cessation for individuals served.
Required Qualifications
- Associate’s Degree in Nursing or related field, or state certificate
- One year of experience working with population served in a nursing capacity
- Current state LPN or LVN licensure for state of residence
- Current driver's license, car registration, and auto insurance
- Certifications such as CPR/FA and HIV/AIDS certification, BBP, ACT training as required by state licensing regulations
