Job Summary
Provide direct patient care including specialized end-of-life duties, implement nursing practice plans, and document services accurately. Assess patients and families for education on diagnoses and self-care, manage pain and symptoms, and evaluate treatment responses. Coordinate hospice services by reporting needs to interdisciplinary groups and use aseptic techniques for equipment preparation. Participate in quality assessment teams and maintain comprehensive knowledge of information systems. Requires current LPN/LVN licensure, three years experience, and flexibility in work hours.
Required Qualifications
- Must currently be licensed as an LPN/LVN in the State
- Must have three (3) years nursing experience
- Must be flexible in work hours
- Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
Desired Qualifications
- community health/hospice or medical/surgical experience is preferred
- Must understand the philosophy of hospice concept and understand needs of the terminally ill
