Job Summary
Provide direct patient care including specialized end-of-life duties, implement nursing practice plans, and document services accurately. Assess and educate patients and families on diagnosis and self-care, manage pain and symptoms, and evaluate treatment responses. Coordinate hospice services with the interdisciplinary team and report identified needs to supervisors. Use equipment adhering to aseptic technique and participate in quality assessment and performance improvement teams. Maintain comprehensive knowledge of information systems and applicable software programs. Requires current LPN/LVN licensure, three years of nursing experience, and flexibility in work hours.
Required Qualifications
- Must currently be licensed as an LPN/LVN in the State
- Must have three (3) years nursing experience
- Must be flexible in work hours
- Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
Desired Qualifications
- community health/hospice or medical/surgical experience
- Must understand the philosophy of hospice concept and understand needs of the terminally ill
