LVN/LPN or RN (PRN)

Conduct daily resident rounds, administer medications per physician orders, and document all care provided. Identify safety hazards, manage pain assessments, and initiate corrective action for resident problems or emergency situations. Provide supervision to direct care associates, perform basic nursing skills, and ensure proper infection control procedures. Review Glucometer Control Logs, count narcotics, and coordinate discharges with physicians and ancillary providers. Listen to resident and family concerns, report changes in condition to the physician and supervisor, and follow community admission and transfer procedures. Maintain accurate clinical records and ensure proper diet service while complying with all nursing standards and policies.
