Job Summary
Provide direct patient care including specialized end-of-life duties, implement nursing practice plans, and document patient services accurately. Assess and educate patients and families on diagnosis and self-care plans while coordinating hospice services with interdisciplinary teams. Manage equipment supplies using aseptic techniques, evaluate patient responses to treatments, and participate in quality assessment and performance improvement teams. Perform other duties as assigned by the registered nurse. Requires current state licensure, three years of nursing experience, and flexibility in work hours.
Required Qualifications
- Must currently be licensed as an LPN/LVN in the State
- Must have three (3) years nursing experience
- Must understand the philosophy of hospice concept
- Must understand needs of the terminally ill
- Must be flexible in work hours
- Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
Desired Qualifications
- community health/hospice or medical/surgical experience
