Job Summary
Provide direct patient care including specialized end-of-life duties, implement nursing practice plans, and document services accurately. Assess and educate patients and families on diagnosis and self-care, manage pain and symptoms, and evaluate treatment responses. Coordinate hospice services with interdisciplinary teams and maintain aseptic techniques while preparing equipment. Participate in quality assessment teams and personal professional development. Perform other duties as assigned by the registered nurse.
Required Qualifications
- Must currently be licensed as an LPN/LVN in the State
- Must have three (3) years nursing experience
- Must understand the philosophy of hospice concept and understand needs of the terminally ill
- Must be flexible in work hours
- Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs

- community health/hospice or medical/surgical experience
