LVN (Hospice) - Quality Assurance

Job Summary
Review documentation for compliance and appropriateness of care, ensuring quality control of all health services provided by the Agency. Prepare forms for pre and post payment audits and conduct one-on-one in-services for documentation problems as needed. Review nurses' notes for completeness, physician order compliance, authorized services, and regulations; verify POC and re-certification accuracy, order updates, frequency updates, goal updates, medication profiles, and care plans. Coordinate patient care with other disciplines. Requires associate degree, one year hospice or home health experience, current State LVN license, driver's license, and ability to travel with reliable transportation.
Required Qualifications
- Associate degree (A.A.) or equivalent from a two (2) year college
- At least one year's experience in hospice, home health or a related field
- Current State LPN or LVN license
- Ability to speak and write effectively
- Ability to work well with other disciplines to coordinate patient care
- Current State driver's license
- Clean background and drug screen
- Ability to travel - must have reliable transportation
