Job Summary
Assist the Registered Nurse in carrying out the plan of care and performing specialized procedures as directed under physician's orders. Prepare clinical and progress notes, keep the Case Manager informed of changes during visits, and reinforce teaching to patient and family members regarding end-of-life care. Actively participate in team conferences and collaborate with the IDG to ensure the patient's plan of care remains current. Perform on-call duties including weekends and holidays per policy and agency need. Weekend schedule includes Saturday and Sunday shifts from 12 AM-8:30 PM or 3:30 PM-12:00 AM. Must hold a current unrestricted LVN license with one year of hospice experience and be willing to travel throughout the Bay Area.
Required Qualifications
- Current and unrestricted LVN license
- 1+ years of Hospice Experience
- Current AHA CPR certification
- Driver's License
- Able to travel to our Oakland office for weekly IDG meetings
- The ability to communicate well, both verbally and in writing
- Weekend Schedule : Saturday + Sunday from 12 AM-8:30 OR Saturday +Sunday from 3:30 PM- 12:00 AM
