LVN - Home Health

Oversee and deliver quality, patient-centered care in patient residences while following the Plan of Care directed by the RN Case Manager and PCP. Communicate necessary revisions regarding patient status, needs, safety, and discharge planning to the RN Case Manager and Clinical Manager. Provide complete, accurate, and timely documentation consistent with the plan of care. Regularly consult, advise, and reevaluate patient status with the patient, family, and other team members as appropriate. Requires 2 years of LVN experience, a valid license, IV Therapy/Blood Draw certification, and proficiency with Microsoft Office. Kinnser (WellSky) EMR and Allscripts experience are preferred. Mileage and driving time are reimbursed.
