Job Summary
Prioritize work activities to achieve optimal patient care for geriatric, adult, adolescent, pediatric, and newborn populations. Document care accurately on patient records, initiate nursing orders for at-risk patients, and administer medications while maintaining an error rate below 5%. Communicate essential status changes to the Director, co-assigned nurse, and Registered Nurse, and delegate tasks to unlicensed personnel under direct supervision. Ensure compliance with all regulatory bodies and National Patient Safety Goals while participating in quality assurance activities. Work within the LVN scope of practice as defined by the California Board, assisting in unusual clinical situations and supporting staffing needs on short notice.
Required Qualifications
- current licensure as a Licensed Vocational Nurse in the state of California
- current BLS certification
- Crisis Prevention Intervention (CPI) certification or obtain within one (1) year of assignment to department
- IV and blood withdrawal Certification or obtain within 1 year of hire date
- Minimum of least two (2) years experience as a LVN in subacute and acute care setting preferred
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals
- Ability to write routine reports and correspondence if required
- Ability to calculate figures as it pertains to medication administration if required
- basic computer skills
- Must be able to work normal/scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call
- Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours
- As a designated disaster service worker you are required to assist in times of need pursuant to the California Emergency Services Act
- Needs to recognize that they have an affirmative duty and responsibility for reporting perceived misconduct, including actual or potential violations of laws, regulations, policies, procedures, or this organization's standards/code of conduct

- Adheres to hospital standards, policies and procedures relative to situation
- Available and functions as a resource to other caregivers
- Demonstrates the value of teamwork by relating to co-workers in a collaborative and supportive manner
- Demonstrates adherence to the National Patient Safety Goals for Hospitals as defined by scope of practice
- Delivers patient care with compassion, empathy and respect utilizing the nursing process model
- Under direct supervision of the R.N., identifies pertinent patients' needs/problems
- Is responsible for the treatment of care appropriate to the ages of the patients served
- Under direct supervision of the R.N. Participates in ongoing revision of individualized patient care plan identifying measurable long/short-term goals and discharge plans
- Initiates physician's verbal/written ordered services
- Delivers basic nursing care utilizing policies, procedures and standards of care
- Evaluates patient response to specific nursing interventions
- Assumes responsibility for continued professional growth through participation in continuing education opportunities
- Attends unit, hospital and other available in-service programs according to education policy
- Fulfills work schedule commitment (attendance)
- Dresses appropriately
- Attends unit meetings
