Job Summary
Deliver patient care to residents aged 21 and older in a subacute setting, prioritizing work activities and initiating nursing orders to ensure safety. Document care accurately on patient records, administer medications with a rate less than 5% error, and practice appropriate infection control techniques. Communicate essential information to the Director and Registered Nurse regarding significant patient status changes, while participating in quality assurance activities and in-service programs. Work within your scope of practice under RN supervision, delegate tasks to unlicensed personnel, and adhere to National Patient Safety Goals. Maintain availability for scheduled hours including holidays, weekends, and on-call rotations, as well as emergency and disaster response per the California Emergency Services Act.
Required Qualifications
- Six (6) months of Prior LVN experience
- Must be able to work normal/scheduled working hours to include Holidays, call-backs, weeknights, weekends, and on-call
- Agrees to participate, as directed, in emergencies and community disasters during scheduled and unscheduled hours
- Must have and maintain current licensure as a Licensed Vocational Nurse in the state of California
- Must have and maintain current BLS certification or obtain within orientation process, prior to hands-on patient care
- Must have IV and Blood Withdrawal Certification or obtain within 1 year of hire date
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals
- Ability to write routine reports and correspondence if required
- Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, and percentages if required
- Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form
- Ability to deal with problems involving several concrete variables in standardized situations
- Basic computer skills
- Ability to recognize that they have an affirmative duty and responsibility for reporting perceived misconduct, including actual or potential violations of laws, regulations, policies, procedures, or this organization's standards/code of conduct
- Ability to work well under pressure
- Ability to meet multiple and sometimes competing deadlines

- Ability to assist in obtaining general patient information
- Ability to encourage patient, family, significant other participation in identification of needs, i.e. physical, emotional, educational, discharge, etc.
- Ability to identify pertinent patients' needs/problems under direct supervision of the R.N.
- Ability to be responsible for the treatment of care appropriate to the ages of the patients served, i.e. Adolescent, Adult, and Geriatric
- Ability to obtain and interpret information in terms of the patient's needs
- Ability to possess knowledge of growth and development
- Ability to have an understanding of the range of treatment needed for these patient groups
- Ability to participate in ongoing revision of individualized patient care plan identifying measurable long/short-term goals and discharge plans under direct supervision of the R.N.
- Ability to initiate physician's verbal/written ordered services
- Ability to attempt to ensure patient understanding of special preparation for procedures performed in and by other departments to facilitate a coordinated effort toward optimal patient care
- Ability to deliver basic nursing care utilizing policies, procedures and standards of care
- Ability to accurately report and record assessments, patient data, medication, treatment, procedures and contribute to evaluation of progress towards goals
- Ability to evaluate patient response to specific nursing interventions
- Ability to assume responsibility for continued professional growth through participation in continuing education opportunities
- Ability to attend unit, hospital and other available in-service programs according to education policy
