LVN/Complex Needs

Provide direct care to patients with complex medical needs, including those with chronic conditions, disabilities, or acute illnesses. Monitor vital signs, administer medications, and assist with activities of daily living while ensuring patient safety. Collaborate with the interdisciplinary team to develop and implement individualized care plans. Maintain accurate documentation of patient progress and communicate updates to the healthcare team.
