Job Summary
Conduct nursing assessments, coordinate admissions, and educate patients and families about hospice and palliative care. Review consents and other documents to assist with the election of hospice benefits. Collaborate with interdisciplinary teams to coordinate care transitions from hospitals and facilities. Complete documentation using EMR systems. This field-based position serves as a liaison between patients, families, referral sources, and the hospice/palliative care team in the Long Beach-South Bay areas. Required Qualifications
- Current California LVN License
- 1+ Year Nursing Experience
- Strong Communication Skills
- Reliable Transportation
- Valid Driver's License
