Job Summary
Assess member needs across physical, mental, and social health domains to develop care plans and set SMART goals. Coordinate health benefits, advocate with providers, and connect clients to community services and transportation. Conduct outreach, document progress in the EMR and CRM system, and complete monthly compliance reporting. Utilize evidence-based practices including Motivational Interviewing and Trauma-Informed Care principles. Maintain up-to-date patient records and attend assigned training. This hybrid role requires traveling throughout Orange County up to five days per week; candidates must reside within 20 miles of the territory.
Required Qualifications
- Active, unrestricted LVN license in California
- 2+ years of experience as a care manager, care navigator, community health worker, or home health nurse supporting vulnerable populations
- Willing and able to work Monday-Friday 8:30am-5:00pm, both in the field and remotely, with flexibility for potential evenings and weekends
- Working knowledge of government and community resources related to social determinants of health
- Excellent oral and written communication skills
- Positive interpersonal skills
- Clean driving record, valid driver's license, and reliable transportation
- Must have general computer skills and a working knowledge of Google Workspace, MS Office, and the internet
- Must reside within 20-miles of the assigned territory
- Must be vaccinated against influenza
- Compliance with federal law requires identity and work eligibility verification using E-Verify upon hire
