Job Summary
LVN Case Manager position in Bakersfield (temporary assignment) focusing on Enhanced Care Management for members with complex medical and social needs. Responsibilities include caseload management, comprehensive in-person assessments, development of person-centered care plans, coordination with inter-disciplinary teams, arranging resources and services (health, LTSS, housing), monitoring progress, conducting transitional/discharge planning, coordinating medication review and transportation, and maintaining clear documentation and communication with care teams. Requires CA LVN license, 2+ years healthcare experience, strong communication, and ability to work in field-based, in-person settings. Schedule typically Monday–Friday 8am–5pm with benefits and hourly pay.
Required Qualifications
- Education: High School diploma or GED
- Licensure: Current California LVN license
- Experience: Minimum of two (2) year experience working in a health care setting; managed care or IPA experience preferred
- Demonstrated knowledge of nursing processes, case management, and continuity of care
- Ability to respect the needs of members, support givers, team members, and others, and provide excellent customer service
- Willingness to collaborate as part of a team with professionals at all levels to achieve goals and remove barriers to member health
- Strong communication skills (verbal and written)
- Proficiency in creating professional documents with proper grammar and punctuation
- Reliable in attendance and adherence to work schedule
- Ability to maintain confidentiality
- Ability to travel to locate and engage with members in the field
- Possession of valid driver’s license and auto liability insurance
- Evidence of ability to work independently and coordinate multiple tasks
- Experience with ECM populations (homeless, SMI, SUD, complex conditions, incarceration transition, child welfare, birth equity)
