Job Summary
LVN for Home Health serving Salida, CA in San Joaquin County area. Provide patient-centered care in patients’ residences, following the Plan of Care directed by the RN Case Manager and PCP. Regularly communicate patient status or needs to the RN Case Manager and Clinical Manager. Monitor and reevaluate patient status with patient, family, and care team regarding safety, plan, and discharge planning. Deliver complete, accurate, and timely documentation consistent with the plan of care. Mileage reimbursement and driving time paid; flexible schedule; benefits including medical, 401k matching, CEU, and ongoing support.
Required Qualifications
- LVN license required
- 2 years LVN experience, home health preferred
- IV Therapy/Blood Draw certification required
- Proficient in Microsoft Office (Word, Excel, PowerPoint, Outlook)
- Experience with Kinnser (WellSky) EMR preferred
- Experience with Allscripts preferred
