Job Summary
Conduct clinical assessments and medication reconciliation for clients, then develop, implement, and evaluate Individualized Care Plans (ICP). Serve as a resource and advocate for Members while acting as a liaison between clients, caregivers, and providers. Organize Interdisciplinary Care Teams to ensure facilitation and documentation, and assist with referrals for specialized programs like CCS and multi-organ transplant. Support clients with immediate needs regarding access, authorizations, and resources for Long-Term Services and Supports. Review and revise contributions from care team members to ensure successful care management processes and Member outcomes.
Required Qualifications
- Valid, unrestricted RN license issued by the State of California
- Possession of a valid State of California driver's license
- Minimum 2 years clinical experience in an acute care facility, skilled nursing facility, home health or clinic setting
Desired Qualifications
- Two (2) or more years of care management experience in a health care delivery setting
- Experience in a Managed Care (HMO, IPA) or in acute facility (i.e. hospital) care management
