Job Summary
Coordinate patient care as the primary point of contact for assigned patients within a transdisciplinary UTC program; assist patients with social service needs; participate in peer-to-peer or care management reviews; ensure UTC scheduling and prep for presentations; present patients to provider teams during care reviews; provide nursing services including assisting with examinations and procedures, administering medications and injections, checking blood glucose, processing referrals, and relaying provider instructions; document encounters in the health record system; maintain clinic kits and safety equipment; complete patient follow-ups and outreach; engage patients in self-management and use of community resources; be on-call after hours as needed; bilingual English/Spanish preferred.
Required Qualifications
- Current Vocational Nurse license in the state of California
- High School Diploma or equivalent
- Basic Life Support (BLS) certification
- General computer skills including typing, Microsoft Word, Microsoft Excel, Microsoft Outlook, and medical record data review and entry
