Job Summary
Receive case referrals, review patient information to determine home care needs, and assign appropriate clinicians to cases. Evaluate each case by reviewing services provided, conferences, and records; instruct clinicians to promote effective performance and delivery of quality home care services. Review patient clinical diagnosis, medications, procedures, and clinical course. Assist clinicians in establishing therapeutic goals, setting priorities, and developing plans of care. Attend case conference meetings, conduct quarterly record reviews, and communicate findings to the Clinical Director. Assist in screening new personnel, orienting new hires, and planning continuing education programs. Formulate and evaluate policies, procedures, and strategic goals while promoting customer service orientation. Comply with professional standards and perform other delegated duties.
Required Qualifications
- Licensed vocational nurse with current licensure to practice professional nursing in the State
- Two years case management experience within the past five years in a home health agency
- Knowledge and expertise in clinical decision-making and Quality Management review and evaluation
- Complies and maintains current CPR certification
- Demonstrates excellent observation, verbal and written communication skills
