Job Summary
Review patient information and case referrals to determine home care needs, then assign appropriate clinicians and establish therapeutic goals. Evaluate services provided by clinicians through conferences and record reviews, instructing staff to promote effective performance and quality delivery. Attend case conferences and conduct quarterly record reviews to communicate findings to the Clinical Director. Assist in screening new personnel, orienting hires, and developing inservice education programs while formulating policy revisions. Coordinate care across disciplines, maintain CPR certification, and comply with professional standards to ensure appropriate service delivery.
Required Qualifications
- Licensed vocational nurse with current licensure to practice professional nursing in the State
- Two years case management experience within the past five years in a home health agency
- Knowledge and expertise in clinical decision-making and Quality Management review and evaluation
- Complies and maintains current CPR certification
- Demonstrates excellent observation, verbal and written communication skills

- Nursing degree is preferred
