Job Summary
Receives case referrals, reviews patient information to determine home care needs, and assigns appropriate clinicians. Evaluates services provided by clinicians, discusses impressions, and guides staff to promote effective quality home care delivery. Reviews patient clinical diagnoses, medications, and procedures while assisting in establishing therapeutic goals and developing care plans. Attends case conferences, conducts quarterly record reviews, and communicates findings to the Clinical Director. Assists in screening new personnel, orienting staff, and planning continuing education programs. Formulates policies and procedures, complies with professional standards, and participates in community activities. Requires current LVN licensure, two years of home health case management experience, and CPR certification.
Required Qualifications
- Licensed vocational nurse with current licensure to practice professional nursing in the State
- Two years case management experience within the past five years in a home health agency
- Knowledge and expertise in clinical decision-making and Quality Management review and evaluation
- Complies and maintains current CPR certification
- Demonstrates excellent observation, verbal and written communication skills
