LTSS Svc Coord-RN Clinician (US)

Manage member cases within licensure scope by conducting clinical assessments, developing individual care plans, and optimizing health care across the continuum. Provide direction and oversight to non-RN clinicians, including LPNs, LCSWs, and LSWs, by assigning tasks, verifying information, and evaluating performance. Coordinate care for members with chronic illnesses, co-morbidities, and disabilities while facilitating authorizations and referrals. Obtain thorough member histories, establish short and long-term goals, and identify candidates for alternative care levels or waiver programs. Interface with Medical Directors and interdisciplinary teams to solve provider and claims issues. Requires an active RN license and 3+ years of experience in service coordination or case management.
