Job Summary
Develop and implement personalized care plans for dual-eligible beneficiaries while coordinating services among healthcare providers, social services, and community resources. Facilitate effective communication between all parties involved in the patient's care and advocate for their needs to ensure appropriate and timely access to Medicare and Medicaid. Assist patients and families in navigating the complexities of these programs, connect them with long-term services and supports, and maintain accurate documentation of all interactions. Ensure all activities comply with relevant regulations and organizational policies. Work collaboratively with interdisciplinary teams to deliver holistic care and monitor the effectiveness of provided services.
Required Qualifications
- BA/BS
- minimum of two years working with social work agency, LTSS, healthcare, and/or aged and disability industry experience
- Experience in care coordination, case management or related role within the healthcare or social service field
- Experience working with dual-eligible populations
- deep understanding of Medicare and Medicaid programs
