Job Summary
LTC CONSULTANT performs assessment and screening to link individuals to needed services, conducts Level of Care and Comprehensive assessments, maintains documentation per PAA/ODA/Ohio Department of Medicaid standards, collaborates with informal caregivers and formal services, and supports access to services while safeguarding confidentiality. The role requires ability to work from home, travel across a multi-county area to private homes/facilities, and completion of Ohio Aging Network certification within 6 months. Proficiencies include strong verbal/written communication, Word/Excel, and professional, respectful interactions within HIPAA guidelines.
Required Qualifications
- Registered Nurse or Licensed Social Worker holding an active license to practice in the State of Ohio
- Successful completion of the Ohio Department of Aging certification required within 6 months of employment
- Ability to work from home
- Strong verbal and written communication skills
- Knowledgeable in Microsoft Word and Excel
- Ability to communicate professionally, respectfully and effectively
- Ability to make sound decisions and demonstrate critical thinking skills
- Ability to prioritize, organize and maintain accuracy in a flexible job environment
- Travel ability to travel daily in a multi-county service area and access private homes and facilities
