Job Summary
Conduct comprehensive face-to-face or in-home Health Functional Assessments within 15 days of notification to identify special health care needs for adult, senior, and child members. Collaboratively develop client-focused Quest Integration service plans with primary care physicians and decision makers, while coordinating Home Community Based Services and assisting families in crisis. Maintain documentation in the KPHC electronic health record, tracking service plan progress and medical care continuity. Provide resource identification and referrals for dental, social, emotional, behavioral, child welfare, and developmental services, serving as a resource person for members, families, and community agencies. Conduct social work consultation and navigate supplies and equipment with Member Care Services Associates. Requires a Licensed Clinical Social Worker or Licensed Social Worker in Hawaii, one year of social work experience, and Basic Life Support certification.
Required Qualifications
- One (1) year social work experience
- Masters degree in social work OR a minimum six (6) years of experience in a directly related field

- Basic Life Support required at hire from American Heart Association

- Demonstrated knowledge of and skill in decision making
- Demonstrated knowledge of and skill in conflict resolution
Desired Qualifications
- One (1) year of care coordination/case management experience preferred
- Home health experience preferred
- Healthcare experience working with medical diagnoses preferred
- Demonstrated ability in networking with community agencies preferred
