LSW Inpatient/Social Services Honolulu - On-Call

Assess, care plan, and coordinate social work services for patients and families while consulting with staff and health care providers. Initiate social work plans of care and serve as a liaison between Kaiser Permanente and the community to integrate services and coordinate patient placement. Prepare and evaluate assessments, develop service plans, and perform case management, casework intervention, and monitoring of resources and conditions. Provide resource identification, referral, mediation, advocacy, and education to individuals, groups, couples, and families. Assist with administration of social work programs, conduct research, perform clinical record audits, and implement quality improvement processes. Maintain educational records, participate in departmental functions, and orient new personnel. Requires a Masters degree in social work, Hawaii licensure, and Basic Life Support.
