Job Summary
Conduct comprehensive client assessments, develop individualized care plans, and coordinate home and community-based services for long-term clients age 60 and older enrolled in the MyCare Ohio CareSource waiver programs. Advocate for client needs and quality care outcomes while monitoring service effectiveness and well-being. Collaborate with interdisciplinary teams, providers, families, and community partners to improve quality outcomes for older adults and individuals with disabilities. This hybrid field-based role supports clients to remain independent in their homes across Cuyahoga, Geauga, Lake, Lorain, and Medina Counties.
Required Qualifications
- Current licensure as a Licensed Social Worker (LSW) through the State of Ohio Counselor and Social Work Board
- Minimum of one (1) year of experience working with individuals with physical or behavioral disabilities in home healthcare, medical social work, or behavioral health environments
- Excellent communication, interviewing, and assessment skills
- Knowledge of community resources available to older adults and individuals with disabilities
