Job Summary
Provide skilled home health physical therapy services under the direction of a licensed Physical Therapist, implementing individualized treatment plans to improve mobility, balance, strength, and functional independence. Perform home visits delivering compassionate, patient-centered care while observing progress, conducting assessments, documenting findings, and modifying interventions as directed. Educate patients and caregivers on home exercise programs, safety techniques, and fall prevention, and collaborate with physicians and other interdisciplinary team members. Maintain accurate documentation ensuring compliance with Medicare guidelines and Virginia regulations.
Required Qualifications
- Active Virginia Physical Therapist Assistant (PTA) License
- Graduate of an accredited Physical Therapist Assistant program
- Minimum of one (1) year of clinical experience as a Physical Therapist Assistant, preferably in home health or another post-acute care setting
- Knowledge of state and federal home health regulations
- Strong clinical judgment, communication, interpersonal, and documentation skills
- Current CPR certification (or ability to obtain)
- Valid driver's license, reliable transportation, and proof of auto insurance
