Job Summary
Conduct advanced clinical home care services including patient assessment, medication management, and patient/family education under RN direction. Perform revisits to patients in their homes based on skilled needs, implementing plans of care and evaluating treatment progress. Accurately observe, measure, record, and report clinical data related to health status, while documenting visits electronically within 24-48 hours. Reconcile and administer prescribed medications and perform nursing treatments such as urinary catheterization and sterile dressing changes as indicated in the physician's plan. Requires NY State LPN license, one year of med/surg experience, and good computer skills.
Required Qualifications
- NY State LPN License
- Minimum of one year Med/surg experience in a hospital or Longterm Care facility
- Good computer skills
